Parents/Guardians Code of Conduct & Responsibility
- I / We will fully support the core values of the Club and School, show respect to, and abide by the decisions of the Coaches and Management.
- I / We will follow the Club’s Standard Operating Procedures (SOP) at all times.
- I / We will make every effort to ensure my son/s attend the weekly training sessions and matches.
- Whenever possible, I / We will attend matches to support my son/s and the team.
- I / We will equip my son with boots or sports shoes and shin guards during every training session and match.
- I / We will conduct ourselves with the highest standard of behaviour throughout every training session and match.
- I / We will submit any complaints to the Club in WRITTEN FORM ONLY.
I/We hereby give permission for the named player in this form (“the player”) to participate with
KMS YOUTH SOCCER FOOTBALL ACADEMY (“the Club”) in training or matches, and declare that I/We:
- will assume all risk of personal injury (including death) and damage to school or personal property sustained and/or suffered by the player during his/her involvement and/or participation in any training session or match.
- agree to release the Club’s Owners, KMS Youth Soccer Football Academy (“the Club”), its employees, and any other party related to organising the training or match (“related parties”), as well as the Ministry of Education, from any and all liability for any injury, death or damages suffered by the player during any training session or event organised by the Club — whether due to the negligence of the Club, its employees, peers and/or related parties.
- will fully and effectively defend, indemnify and hold the Club, its employees and/or related parties harmless from and against any and all actions, suits, claims, demands, causes of action, proceedings, losses, costs and expenses (including, without limitation, all legal fees and disbursements), damages, liability, fines and penalties in any way arising out of, relating to, or connected with — directly or indirectly — the player’s participation in any training session or match, regardless of whether there is active or passive negligence or fault on the part of the Club, its employees, peers and/or related parties.
- Personal Data Protection Act (PDPA) Consent — I/We hereby give consent and authorise KMS Youth Soccer Football Academy to process, including disclosing, my/our personal data and that of my/our children for all purposes relating to the activities of the Club.
Player’s Declaration
I agree to abide by this Code of Conduct and to participate in training sessions and any other events organised by the Club, with the blessing of my parents/guardians. I will follow all regulations and instructions given by the Coaches, commit to attending every training session and event punctually, and ensure that my participation does not affect my studies or preparations for examinations.
Disciplinary Notice
Please note: the Club reserves the right to take disciplinary action against any player or parent/guardian who fails to comply with this Code of Conduct.
